Make an Appointment

Patients are seen by appointment – on the same day the appointment is made if possible. To schedule an appointment, please call (386) 775-0839 during regular business hours.

Please call in advance for office visits. Make follow-up appointments as you leave. We make every effort to stay on schedule although emergencies do arise. If we are seriously delayed, we attempt to notify patients beforehand.

New Patients

New patients are asked to arrive 15 minutes early to complete paperwork. You can download, print, and complete the forms prior to your appointment. Please bring the completed forms, a list of all medications you are currently taking, recent copies of x-rays, lab results and your insurance card to your appointment.

Pap Smears

If you have made an appointment for an annual exam or a pap smear, please do not use vaginal creams or suppositories, tampons, douche, or have intercourse for 48 hours prior to your pap smear. This ensures that we collect the best possible sample.


Orange City OB – GYN accepts adolescent patients from ages 14-17. Please be aware that in order to care for a minor, the patient's parent or guardian must sign a minor consent form giving us permission to treat the patient in a confidential manner. The information contained in the medical record is confidential and will not be given to parents without the patient's consent.

Children’s Policy

While we cherish seeing your children at your visits, we ask that you please bring another adult with you to watch your children in the waiting room so that we can address your medical needs without distraction. If you would like to bring your children into the exam area, again, we ask that you bring another adult with you to watch your children so that they don’t open trash containers or drawers in the exam room which can contain biohazardous materials or expensive equipment.

Medical Form Completion

We receive many forms from patients requiring information and signatures from our office. For forms requiring a signature only, please bring the completed medical form to our office for a signature. If you have applications which need to be completed by Dr. Hoffmeister or the nurses, we charge a modest $25 fee for medical form completion. Please allow 1 week for form completion. Be aware that disability forms will be filled out only if your doctor has limited your work capacity due to a medical problem. We will provide you with a work release when we can document your illness, injury and recovery. We cannot provide forms attesting to matters in which we were not involved.

Medical Records

The office keeps a confidential record of your medical information. No information in that record will be released to anyone without your written permission. This protects you as well as our practice.

If you need copies of your records, reports contained in your record, information for your files, or for your record to be sent elsewhere, you must sign a medical release authorization. The cost to provide copies of your medical records is $1.00 per page up to 25 pages and 25 cents per page thereafter.


We require payment of your co-payment and past-due account balances at the time of service. We accept cash, checks, debit and all major credit cards.

We will submit claims to most major insurance carriers. Please bring your insurance card and I.D. with you to every visit so we can ensure that our records are accurate.

We recommend that you call your insurance company to verify that we participate with your plan.

Please notify us if you have any change in your name, phone number, address, or insurance as soon as it occurs. And please be prepared to present your insurance card at every visit.

If we are not contracted with your insurance company or if you are a self-pay patient, you will be responsible for payment at your visit. We will be happy to provide you with the overall bill that we use so that you can file it with your insurance company.

Cancellations or Rescheduling

To better serve the needs of all our patients, we ask that you call at least 24 hours in advance to cancel or reschedule appointments. We make every effort to stay on schedule, although emergencies arise. It is very helpful if we have a current cell phone or work number so we can notify you as soon as we are aware of a situation that might affect your appointment time. Please notify our front desk if you have been waiting more than 15 minutes past your appointment time.

We understand that unexpected situations do occur causing you to miss your appointment. However, we do reserve the right to charge a $25 fee for missed appointments not canceled 24 hours in advance.

Refill Requests

Due to the high volume of phone calls to our office, we request that you obtain your prescription refills at the time you are in the office. We do not refill medications after hours or on the weekends and rarely without an appointment. We have found that this significantly reduces our call volume so that we can address urgent problems more effectively. If you are requesting a new medication prescription, please make an appointment so that you can discuss this with your physician. He will want to discuss and alert you to possible side effects or interactions of any new medications.

After-Hours & Emergencies

There is a physician on call 24 hours a day, 7 days a week. Please limit after hours call to urgent and emergent problems. Urgent problems are those that cannot wait until the next business day. Non-urgent calls are those that deal with routine questions, lab results, or prescription refills. NO PRESCRIPTION REFILLS WILL BE CALLED IN AFTER OFFICE HOURS. Please call our office early in the day if you have a problem. Calls late in the day will have to be referred to the emergency room. WE DO NOT ACCEPT WALK IN APPOINTMENTS. Please call our office to arrange an appointment convenient to your schedule.
